Association between sensation seeking and HIV risk behaviors using latent class analysis among college students in Zhuhai, China

摘要: 目的了解大学生感觉寻求(sensation seeking, SS)与人类免疫缺陷病毒(human immunodeficiency virus, HIV)高危行为潜在类别的关联，为高校开展HIV高危行为干预工作提供参考依据。方法 2019年10―12月，采用多阶段整群抽样方法收集珠海市6所高校1 430名学生社会人口学特征、SS、HIV高危行为等信息。对性前物质使用、性前饮酒、无保护性行为、临时性伴、商业性伴、多性伴等HIV高危行为进行潜在类别分析(latent class analysis, LCA)，使用多因素logistic回归分析模型分析SS与不同HIV高危行为类别的关联。结果珠海市大学生HIV高危行为分为低危行为组(83.0%)、高危行为组(17.0%)。调整了性别、年级、专业、性取向等因素后，多因素logistic回归分析模型结果显示，SS(OR=1.02, 95% CI: 1.00~1.05, P=0.018)、SS中的去抑制(OR=1.18, 95% CI: 1.08~1.29, P < 0.001)与HIV高危行为类别呈正相关。结论珠海市大学生HIV高危行为存在聚集特征，且高SS的大学生更易进入HIV高危行为组。对大学生进行HIV和性健康教育时，应结合HIV高危行为潜在类别特征及SS特质制定针对性干预措施。

Abstract: Objective To examine the relationship between sensation seeking and the latent classes of human immunodeficiency virus (HIV) risk behaviors in college students, and to provide suggestions for the intervention of HIV risk behaviors in colleges. Methods Between October and December 2019, a multistage cluster sampling method was used to collect data on demographic information, sensation seeking, HIV risk behaviors among 1 430 students from 6 universities in Zhuhai, China. Six HIV risk behaviors which includes substance use before sex, alcohol use before sex, unprotected sex, casual sexual partners, commercial sexual partners and multiple sexual partners were analyzed using latent class analysis (LCA). After adjustment for gender, grade, major, sexual orientation and other factors, multivariable logistic regression were performed to explore the association between sensation seeking and the latent classes of HIV risk behaviors. Results College students in Zhuhai were classified into a low-risk behavior group (83.0%) and a high-risk behavior group (17.0%) according to LCA. The multivariable logistic regression analysis showed that higher levels of sensation seeking (OR=1.02, 95% CI: 1.00-1.05, P=0.018) and disinhibition (OR=1.18, 95% CI: 1.08-1.29, P < 0.001) were more likely to be in the high-risk behavior group. Conclusions There was an aggregation of HIV risk behaviors among college students in Zhuhai, and students with higher levels of sensation seeking were more likely to be in the high-risk behavior group. During HIV and sexual health education, targeted interventions should be tailored according to the latent classes of HIV risk behaviors and the trait of sensation seeking.
